Question: Horse isn't doing well... please help :(?

So I bought y horse about three years ago and she was 18. Now 21 almost 22, she's not doing well. She's always had trouble with her weight (always underweight) and at my old ranch we could manage it. I switched ranches about a year ago due to personal reasons. They don't have good grass, almost to the point that it wouldn't even do anything for her. She's supposed to get grain twice a day and hay, but overtime I've gone to the ranch they didn't have any grain in the grain bucket. Also, last time i went out they didn't have any hay. I went out a couple weeks ago and she looked a little more underweight than usual, but its winter so that can be expected. Then, I went out a couple days ago and she looks TERRIBLE!!! I burst out in tears just saying how sorry I was for not being able to be there enough to check on her more often. Im really worried about her and i don't want to get mad at the people that own the ranch, for they are older and don't have much money and the lady is going through operations. I want to get a vet out, buy her her grain myself, and buy her alfalfa hay to help, but she's so skinny I'm so worried. I want to switch ranches that can give her more attention, and i told my parents that but they don't understand. Any ideas for what could help her weight? Any other advice? i am so upset please help me i love her more than anything
