Question: Injured puppy has heavy breathing and his panting can be quite noisey??

Yesterday night my neighbors friend was on her way somewhere and backed into a small (maybe 2-3 months old idk). He has a pretty big gash on his front leg and had trouble walking on one of his rear legs. We think it is only sprained because he doesn't yelp or whine like crazy when I touch or move it. He was obviously malnourished and was a stray, me and my family bathed him, washed his cuts, dressed his wounds, gave him anti-biotics, benadryl and fed and watered him. He hasn't done much moving around today, just resting in the pet cage, he isn't whining or anything. He just seems tired and has been really thirsty. He ate well earlier also. He has just been resting in the cage, breathing heavily and pretty fast as if he had been pretty active, which he hasn't. When he is sleeping the breathing can get noisy as well like he has slightly clogged sinuses. Also I think he may have puppy worms because of his slightly bulgy stomach. I just want to know what you guys might think is wrong and what I should do to ease him and if there might be anything wrong, like could he have internal bleeding or what to look for???? Thank you guys! ***Where I live is in the middle of nowhere, no veternary clinic for miles, I don't have enough money to get him XRays or checked up, I don't think as this can get very expensive I've learned from one of my other small dogs. I would like to keep him as the "shelters" around here are terrible and inhumane because of the number of stray dogs in my area.
