is there something wrong with my dog ?

My dog is in heat she has been in heat before she's had puppies about 6 months ago she has been fine all day and now she's shaking can't seem to get comfortable and just looks in pain what could be wrong with her ? oh and the day before we noticed she was in heat which was 2 days ago she got out and we found her i don't know if she could have got pregnant ? Can dogs have miscarriages and would it happen this soon
